Hospice Regional Manager - Grace Healthcare Services
Location: Middlesex, Union, and Somerset Counties
Job Type: Full-Time
Schedule: Monday - Friday, 8:30 AM - 5:00 PM
Job Summary: The Regional Manager is responsible for assisting the Clinical Director with the development, implementation and evaluation of Grace Healthcare Services hospice care as well as managing all aspects of the interdisciplinary team.
Key Responsibilities:
- Participate in the coordination, planning and delivery of quality and compliant hospice care.
- Evaluate patient services provided by the interdisciplinary team (IDT).
- Review care plans and medication lists for new admissions and review charts.
- Maintain working knowledge of hospice state licensure as well as federal regulations.
- Monitor services and documentation to meet hospice policies, state and federal regulations.
- Serve as liaison to Clinical Director regarding challenges and patient care.
- Supervise and participate in the provision of care provided, orientation, scheduling, education, and support of all caregivers on the hospice interdisciplinary team including any contracted staff.
- Be available for on-call if needed. Provide support to on-call staff.
- Schedule and run meetings of the interdisciplinary team.
- Develop physician relationships in order to familiarize them with hospice care.
- Act as liaison between IDT members, patients, their families, and our customers.
- Contribute to effective patient and customer relationships by assisting patients, families, physicians, and staff to resolve expressed concerns.
- Facilitate interdepartmental communication with co-workers and staff.
- Facilitate, advocate, and validate the role of the volunteer as part of the interdisciplinary team.
- Participate in business development activities such as implementations and presentations.
- Responsible for assisting the program to meet and/or exceed all program goals.
- Responsible for compliance of all team members notes including accuracy and timeliness of all patient related documentation in Brightree.
- Responsible for reviewing all team members' time and activity sheets ensuring accuracy and timelines.

- Complete and submit weekly assignments of the team.
- Communicate care plans based on NANDA International guidelines with a prep note in Brightree after every visit.
- Discipline orders, care plans and medication lists must be entered into Brightree on the same day as IPOC paper submission.
- Review On Call report each morning and ensure appropriate follow up and patient & family immediate needs are met.
- Monitor On Call report for supply and medication shortages in order to meet the needs of the patients.
- Mentor staff on the team and monitor performance of team, complete staff performance evaluations.
- Manage audit list for compliance and minimal exposure of missing/pending documents.
- Participate actively in-service failure recovery in the field.
- Arrange for coverage for staff on vacation or out with illness.
- Adhere to the practice of confidentiality with regard to patients, staff, customers and all aspects of the organization.
- Ensure that Grace Healthcare Services' best practices are incorporated into all job activities/responsibilities.
- Perform other duties as necessary.
Qualifications:
- NJ Registered Nurse License required.
- Bachelor's or associate degree in nursing preferred
- At least one (1) year experience preferably in hospice or home care, with progressive management responsibilities
- Excellent communication and interpersonal skills.
